The Prevention and Diversion team provides support to individuals with complex needs, including justice involvement, disability, mental health challenges, and social disadvantage. A key function of the team is connecting individuals and stakeholders with appropriate services.
This project will focus on reviewing current coordination practices to identify key areas of need within the cohort, alongside gaps in the team’s ability to connect individuals to appropriate services. The project will then develop a structured understanding of available community supports and build a centralised resource repository to strengthen referral practice.
The purpose of this internship is to:
• Review the Prevention and Diversion service data to identify common areas of client need
• Map available internal and external community services, programs, and supports aligned to these needs
• Develop a comprehensive service and resource repository to support consistent, informed responses
